Job Summary/Purpose:

MUSC’s Department of Medicine is seeking an Advanced Practice Provider to be responsible for conducting medical screenings, assessments, examinations, and treatments of patients referred to the Palliative Care service. The Nurse Practitioner or Physician Assistant will work with physicians and team management to assist in direct patient care, case management, practice development, and implementation of policies and procedures focused on patient-centered care models.

Additional Duties and Responsibilities:

Collaborates successfully as a member of the multidisciplinary Palliative Care Team, taking responsibility for quality and continuity of care of assigned patients.
Embodies and serves as an ambassador for the philosophy of evidence-based symptom management and whole patient assessment for patients with serious illnesses referred to the team.
Moves the practice forward in evidence-based management of symptoms.
Works with the interdisciplinary team to facilitate clarification of patient and family goals of care and access to appropriate support models of care, i.e. hospice, rehabilitation, ongoing aggressive treatment.
Completes timely and accurate documentation of all activities in the EPIC EMR system.
Participates in education of non-palliative care providers and promotes culture transformation to reinforce compassion and support for patients within the health system.
Respects patient confidentiality, privacy, and dignity. Provides services in a quick and courteous manner with care and respect for the patient’s individual needs.
Provide expert assessment and treatment of complex symptoms.
Participate in multi-specialty Palliative Care team meetings.
Help educate medical students, residents, nurses, APP, and physician colleagues and other hospital staff about Palliative Care principles and practices.
Precepting may be required.

Physical Requirements:

Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Ability to be qualified physically for respirator use, initially and as required. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Frequently lift, lower, push and pull and/or carry objects weighing 50 lbs (+/-) unassisted, exert up to 50 lbs of force, lift from 36" to overhead 25 lbs. Infrequently work in dusty areas and confined/cramped spaces.
