General Purpose: The Physician Assistant or Nurse Practitioner will provide same day and rapid access care for primary care patients in the Department of Internal Medicine at Yale Health. Yale Health is a Joint Commission-recognized Medical Home. The PA-C or NP will provide quality rapid access care as needed for patients in the context of a patient-centered medical home. They will also provide urgent care in our Acute Care Department as back-up to dedicated AC staff, as necessary. Ensures safe, effective and timely communication and hand-offs to primary care clinicians, according to departmental guidelines and participates in cross-coverage for absent primary care team members The position will work collaboratively with team members to develop and maintain team-based care by participating in team meetings, rotating leadership responsibility and contributing to performance improvement projects. Adheres to departmental and organizational standards of patient service and responsiveness, including but not limited to timely and appropriate communication of test results and timely response to patient messages. Adheres to organizational standards of medical documentation within the Yale Health electronic health record (Epic) including but not limited to, timely completion of notes, appropriate documentation of non-visit-related transactions and appropriate and timely management of Epic in basket. Participate in rotation for weekend and holiday coverage in Acute Care. Abide by Yale Health Code of Conduct and all relevant policies and procedures. Required Education and Experience: Master’s Degree from an accredited Advanced Nurse Practitioner program or graduation from a Physician Assistant (PA) Training Program accredited by the American Medical Association Council on Medical Education. CT ARPN license (or eligible) with ANA certification OR CT PA license (or eligible). Qualifications:
