The Nurse Practitioner (NP) in the Monroe Specialty Clinic at University Health (UH) is a key member of the patient care team. The NP must possess broad knowledge of the clinical specialty area where they provide health care services. This individual will interact effectively with physicians, patients, nurses, as well as other UH staff. The NP will perform direct and indirect patient care duties under the supervision of designated physician staff while adhering to established protocols and institutional policies. Care may be provided on an outpatient or inpatient basis. The NP will work with the patient and care team to provide diagnosis and treatment, develop patient management plans and where applicable, perform routine procedures and assist in surgery and other major procedures. This individual may also assist the physician and/or physicians in conducting rounds in inpatient acute and long–term care settings.
Responsibilities include but not limited to:
Performs general and/or preoperative detailed and accurate histories and physicals
Performs appropriate physical examination
Orders, performs and/or interprets, at least to the point of recognizing deviations from the norm, common laboratory, radiologic, cardiographic, and other routine diagnostic procedures used to identify pathologic processes and to utilize for pre-operative workups where applicable
Develops and implements patient management plans and protocols
Records progress notes on patients
Writes orders for treatments and other clinical therapies
Writes prescriptions as needed
Performs medical review of referrals into service
Makes outgoing referrals to specialty care, including psychosocial needs as applicable
Prepares patients for admission to hospital where indicated
Provides follow up to patient regarding results of diagnostic testing
Instructs and counsels patient and/or family on medical conditions, compliance with a prescribed therapeutic regimen and self-care issues
Involves patient in care planning and answers patient questions
Monitors patient adherence to protocols
Provides emotional support and guidance to patients and/or their families as they deal with their illness
Assures that patients receive appropriate follow up appointments
Assists in the coordination of care across clinical care settings
Assists in managing the patient postoperatively and/or post-discharge in both inpatient and outpatient settings
Assists care team in urgent/emergent patient care situations
Returns calls related to patient care
Completes forms brought to clinic by patients such as FMLA, disability papers, handicap sticker paperwork, and insurance precertification
Performs routine procedures such as providing injections, suturing wounds and lacerations, wound care and management, catheterization, and other procedures as appropriate to service
Assists in surgery and other major procedures where applicable to service; may include posting cases, assuring that patient is prepared for operative procedure, checking that specialized equipment is functional prior to procedure, assisting during procedure and communicating with families during procedure
Assist with patient flow and discharge planning in collaboration with Case Manager
Utilizes electronic health record to document all patient care activities
Participate in education of resident staff, medical students, and other learners rotating through the clinic |
