GENERAL SUMMARY/ OVERVIEW STATEMENT:

Under the medical direction of the Medical Director and in collaboration with one or more physicians provides health maintenance as well as episodic illness or chronic disease patient care to assigned adult and pediatric patients. The Physician Assistant has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care. In addition, the Physician Assistant participates in quality and disease management programs in the Health Center. The Physician Assistant is responsible for the daily management of selected patients and is under the supervision of the Medical Director.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Provides direct care, counseling and teaching to a select patient population. Incorporates the concepts of health maintenance, prevention and promotion into daily practice through patient education and counseling.

Performs physical examinations and provides preventive health measures appropriate to patient needs. Screens for substance abuse, tobacco use, domestic violence, seat belt use, immunization status, nutrition, exercise, history of falls as necessary and initiates appropriate interventions.

Orders, interprets and evaluates appropriate lab and diagnostic tests to assess patients’ clinical problems and health care needs.

Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab and physical assessment findings.

Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.

Records pertinent patient care findings in the LMR medical record in a timely manner. Completes forms and reports for insurance companies and other agencies, as needed.

Sets priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment.

Maintains interdisciplinary approach to planning and communicating patient care information by discussing patient data with appropriate physicians and other health care professionals. Demonstrates awareness of community resources and assists staff, patients and families to effectively use them.

Participates in quality assessment and improvement activities as requested. Under the direction of the Medical Director will participate in quality improvement and disease management programs.

QUALIFICATIONS:

1. Formal education required:

a. Must have successfully completed a Graduate Level, accredited Physician Assistant Program approved by the Accreditation Review Commission on Education for the Physician Assistant

b. Must have 4 years of college level education and bachelor’s degree or equivalent.

c. Board Certification by the National Commission on Certification of PAs is required.

2. PA shall meet the following minimum requirements for work experience:

a. 2 years full time experience as a primary care PA including adult and pediatric experience.

b. Other previous hands on patient care experience may be considered at the discretion of the Medical Director.

3. Specific licensure/certification required

a. NCCPA certified

b. License as a Physician Assistant in Massachusetts

c. Maintain current DEA and Massachusetts Controlled Substance Licenses

d. Complete 100 hours of CME according to national and state regulations during each 2 year period of employment.

e. Maintain current BLS/ACLS certification

f. Complete BWH Professionalism Program within 3 months of date of hire.

4. Specific Additional qualifications required for the PA position

a. Experience working in an ethnically culturally and racially diverse work staff preferred.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

Bilingual Spanish/English required.

Completes minimum CEU requirements.

Attends all required meetings, in-services and professional trainings.

b. Must exercise the highest degree of professional judgment under the direct supervision and guidance of a licensed physician.

c. Must maintain a professional attitude towards patients and exercise such mature independent originality in solving problems not covered by existing guidelines, within legal limits, to meet the needs of the patients.

d. Must maintain a professional attitude towards patients at all times and an excellent and collegial working relationship with other personnel involved in patient care.

e. Must demonstrate understanding of procedures, policies and documentation required to ensure compliance with hospital standards of care.


3. Direct Care skills/competencies required to provide care appropriate to the age of the patients served on his/her assigned unit.

a. Demonstrate competency to effectively diagnose, formulate appropriate treatment plan, and efficiently implement the plan with an appropriate level of independence from the attending physician.

b. Clinical skills and knowledge as well as professionalism of the candidate are assessed by SJPHC Attending, under the direction of the SJPHC Medical Director.
