General Summary

The Midlevel Provider will function in a multi-dimensional role in the delivery of evidenced based care for the Palliative Care and Chronic Disease population. The Midlevel Provider will serve as educator and expert resource for patients, family members, nursing, ancillary staff and to the community. He /she will serve as a transitional liaison person for the patient from admission to discharge complete with follow-up care.

Essential Duties:
Responsibilities will include rounding and providing direct evidence based clinical palliative care services and medical management for established patients from the Chronic Disease Service. This may include but is not limited to patients with diagnosis of PAH, Heart Failure, COPD or Stroke. This management may include patient history and physical, ordering and interpreting tests, independent and inter-dependent clinical decision making, leadership responsibilities, and making diagnosis in collaboration with attending and/or medical director. The Midlevel Provider will serve as the liaison between patients, families, multi-disciplinary team members and community resources.

Standard Qualifications:

Required: Current Nurse Practitioner or Physician Assistant license through the State of Michigan; BLS; ACLS; Masters degree;
Preferred: Minimum 2 years recent palliative care experience with current palliative care certification, or eligible for certification in palliative care.
Schedule: Weekdays 10a-6:30p, every third weekend.
